Health Quest and Western Connecticut Health Network have combined to form a new nonprofit health system. The name for the new health system will be Nuvance Health. The new health system was created to provide communities across New York’s Hudson Valley and western Connecticut with more convenient, accessible and affordable care.

Summary:
Provides assistance to clients of Nuvance Health Caregivers at Home(NHCH) , with activities of daily living (ADLs) and instrumental activities of daily living (IADLs) as directed on clients’ individual care plans, within the guidelines of the certified nursing assistant (CNA) scope of practice and according to the NHCH policies and procedures.
Responsibilities:
Assists the client with personal care activities including bathing, oral hygiene, feeding and dressing.
Assists the client with exercises, ambulation, and transfer activities. This includes knowledge in use of devices such as canes, walkers, crutches, and lifts.
Independently assists the client with meal preparation/cooking and/or feeding. Must have working knowledge of special diets, diet restrictions, and techniques to maintain sanitary kitchen conditions to prevent food-borne illnesses.
Assists the client with medication prompts to remind the client to self-administer his/her medications.
Identifies and reports changes in client condition.
Independently provides basic homemaking and laundry assistance for client in the home.

Required:
Certified Nursing Assistant, or any person who has successfully completed a course or courses comprising not less than one-hundred (100) hours of nurse aide training, or seventy-five (75) hours of theoretical and clinical instruction in the fundamental skills of nursing in an approved practical nursing or registered nursing education program
