This is an exciting opportunity to join the Mass General Brigham Population Health department as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) focused on conducting Annual Wellness Visits (AWVs) for at-risk patients. The Advanced Practice Provider will see patients that are aligned to Mass General Brigham's PCPs. This APP will play a vital role in assessing and supporting patients, providing education on the value of preventative care, and re-engaging these patients with primary care to achieve better outcomes.
As part of this initiative, the APP will play a key role in 1) Conducting Annual Wellness Visits to identify and address health risks early. 2) Performing comprehensive clinical assessments and ensuring timely preventive care interventions. 3) Engaging patients in health education, goals-of-care discussions, and chronic disease management. 4) Coordinating care with primary care providers and specialists to optimize health outcomes and reduce unnecessary hospital utilization. This role is an essential part of Mass General Brigham's ongoing commitment to improving care delivery for high-risk populations and ensuring proactive, patient-centered management.
We are seeking a full-time, 40-hour Advanced Practice Provider with weekly schedules consisting of 36 hours of clinical, patient-facing time, and 4 hours of administrative time. This will be a hybrid role with the expectation that the APP will work on-site in a Boston/Boston metro area Primary Care clinic up to 2 days per week and remotely for the remainder of the week. Job Summary
The Nurse Practitioner (NP) is a licensed provider. The NP is responsible for the assessment and management of various populations of patients, medical and/or surgical, including diagnostic and therapeutic interventions, development of appropriate plans of care and ongoing evaluation. May also perform additional duties, such as precepting a small group of learners.

Essential Functions
-Provides direct care, counseling, and teaching to a designated patient population in the ambulatory, inpatient, operative, and/or procedural setting.
-Performs complete histories and physical examinations.
-Orders, interprets, and evaluates appropriate laboratory and diagnostic tests.
-Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings.
-Orders medications and writes prescriptions according to organizational and regulatory policies and procedures.
-Consistently provides high quality and timely documentation including admission and progress notes, procedure notes, operative notes and discharge summaries.
-Performs bedside procedures as are appropriate to the patient population.
