About the Role
The Senior Community Care (SCC) program is a longitudinal, integrated care delivery program focused on coordinating clinical care for members in facility settings. As an Advanced Practice Clinician (APC), you will serve our highest-risk health plan members while working within an interdisciplinary team.

Primary Responsibilities
Conduct comprehensive assessments
Manage acute and chronic medical and behavioral conditions
Ensure accurate documentation, including ICD-10 coding
Collaborate with an interdisciplinary care team
Lead advanced illness and advanced care planning conversations
Provide counseling and education to patients and caregivers
Required Qualifications
Active, unrestricted license in the state of residence (or ability to obtain before start date)
Education:
NP: Graduate of an accredited Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) program
PA: Graduate of an accredited Physician Assistant degree program
Certification:
NP: Board certified through AANP or ANCC in one of the following specialties:
Family Nurse Practitioner
Adult Nurse Practitioner
Gerontology Nurse Practitioner
Adult-Gerontology Acute Care Nurse Practitioner
PA: Board certified by NCCPA
Active DEA licensure/prescriptive authority
Physical capability to lift 30 lbs., navigate various dwelling conditions, and perform assessments
Access to reliable transportation
Ability to obtain a collaborative practice agreement (if required in the state)
Full COVID-19 vaccination (in accordance with regulations & client requirements)
Drug-free workplace compliance (Candidates must pass a drug test prior to employment)
Preferred Qualifications
1+ year of clinical experience (long-term care preferred)
Experience addressing complex behavioral, social, and functional health needs
Geriatric & chronic illness expertise
Strong knowledge of advanced illness/end-of-life care
Proficiency in medical documentation, including EMRs
