Mass General Brigham Integrated Care is a system-led operating entity formed by Mass General Brigham to develop new high quality, low cost, innovative community-based ambulatory care.This work stems from Mass General Brigham's unified system strategy to bring health care closer to patients while lowering total health care costs. The Opportunity: Advanced Practitioner - Family Medicine
Why This Role Is Important
As a key member of the clinical team, the Advanced Practitioner will provide acute, episodic care to our primary care patients. Patient care will consist primarily of same-day sick visits with some overflow of physicals and hospital follow-up appointments provided on-site and virtually. This individual should be comfortable with change and interested in working in an evolving environment where tools to provide care are frequently updated and improved.
What You'll Work On
Diagnosing and treating patients in-person and virtually, including recommending suitable treatment plans and considering cost-effective treatment modalities
Collaborating with physicians in managing acute medical needs of patients. Maintaining continuity of care between physician visits.
Maintaining and reviewing patient records, charts and other pertinent information. Posting tests and examination results.
Counseling for each patient on a wellness plan; providing medical and wellness education (facilitated by members of the Integrated Care team)
Prescribing medication in accordance with Massachusetts statute and professional practice guidelines
Collaborating with specialty care as appropriate
Reviewing incoming reports (e.g. lab, x-ray, EKG) and following up in a timely manner
Participating in peer review, quality assurance, provider meetings, and other clinical meetings
Assisting in updating/implementing protocols and principles of practice as requested
Qualifications
About You
BSN, MSN Required
Successful completion of an approved Nurse Practitioner program
Certificate of registration from the Commonwealth of Massachusetts and New Hampshire Board of Registration as a Nurse Practitioner
Minimum of two years of Nurse Practitioner experience in primary care, urgent care, or emergency medicine preferred
OR
Graduate of an accredited American Medical Association-approved Physician Assistant training program
Massachusetts and New Hampshire Physician Assistant Licensure
Minimum of two years of Physician Assistant experience in primary care, urgent care, or emergency medicine preferred
o Monday: Half day virtual appointments followed by half day administrative time
(can be remote)
o Tuesday-Friday: 8:00 AM to 5:00 PM patient-facing, clinical time
