Job Description:



The Director of Clinical Services is responsible as a member of the clinical management team to the Administrator for the implementation, delivery, coordination and supervision of hospice services delivered consistent with policies and procedures. Recognizes and performs duties which need to be performed although not directly assigned. Duties fall within the scope and role as defined by appropriate regulatory agencies and governing bodies.



Licensure: RN


Specialized Skills:




  • Minimum of five years of nursing experience with at least 2 years in supervisory or management role

  • Demonstrated skills in administrative performance, including expertise in written/oral communication, supervision of staff. documentation requirements, service coordination and case management.



Previous Hospice Experience required (Home health experience will not qualify)

E.Responsible for the planning, implementation, delivery and supervision of direct clinical services provided to patients and families; coordination of the Hospice program and the effective and efficient use of resources.



•Communicates all pertinent clinical and administrative information to the Administrator and all team members.


•Arranges team coverage for on-call and staff time off.


•Assigns patient/family to appropriate staff members.


•Reviews assignments and revises as needed, in collaboration with the Administrator to maintain appropriate caseloads.


•Participates and provides leadership in the team for regular IDC team meetings.


•Reviews and revises the patient/family care plan.


•Facilitates problem solving sessions and delivers team support.


•Implements and monitors compliance with agency policies and procedures related to clinical services


•Serves as a member of the clinical management team in order to address problems associated with the delivery of clinical service; Identify staff development needs; put forth suggestions for appropriate changes in policies and procedures; participate in Performance Improvement activities; gives peer input to enhance and improve the middle management team.


•Supervises the maintenance of medical records assuring accuracy, completeness and compliance with licensing regulations, certification standards, legal and ethical standards.


•Collects and reviews pertinent clinical data at team meetings.


•Revises care plans, completes care plans and charts of discharged patients.


•Monitors documentation of all team nurses.


•Recognizes and perform other duties which need to be performed and as assigned by management.



F.
Oversees supervision of clinical personnel.



•Participates in staff selection and orientation of new personnel as appropriate to team responsibilities.


•Conducts supervisory visits to allow for observation of clinical performance of assigned team members.


•Submits written performance evaluations in accordance with policy requirements.


•Provides information for inclusion in written performance evaluations for all staff assigned in team in cooperation with appropriate members of the clinical management team.


•Provides leadership and counsel to clinical personnel to enhance staff development.


•Identifies need for staff education and assists in the development of in-service programs for the IDC team.


•Assists agency in education/training programs.


•Recognizes and perform other duties which need to be performed and as assigned by management.
