Job Description:

Summary: The Patient Care Navigator II works under the direction of the Director and has responsibilities of implementation of workflow processes to the high risk breast care services and Cancer treatment planning within the Breast Care program. Patient navigation to HRA breast MRI imaging, interventional imaging, osteoporosis, and downstream to ancillary cancer services, such as, Cancer Risk and Prevention Program, treatment planning and associated activities. The position is responsible for navigating the daily patient flow, capturing all clinical recommendation and referral resulting in downstream patient care services through navigation workflows. Daily responsibility include extracting system reports confirming clinical recommendations and/or referrals to the  breast services, contact and educate patients and referring office of  high risk analysis documentation through the HRA breast algorithm tools and Cancer Risk and Prevention Program (CRPP), and related recommendations leading to referrals, insurance authorization, treatment planning of patient visits and service care support. Responsibilities include provision of organization, communication and assertive skills necessary to facilitate patient activities in coordination with ancillary departments. Identifies and resolves discrepancies related flow process of patient care. Also, ensures adequate inventory of medical and office supplies. Accountability include collaboration with the healthcare team towards achieving fiscal, quality care goals, and data outcomes for breast care program services and Cancer Risk and Prevention Program.

Qualifications:


  • Knowledge of Medical Terminology , ICD/CPT coding


  • Five years experience in the Healthcare Industry, required


  • Proficient in computer systems, software applications, and data entry


  • Back office medical assistant experience preferred


  • High School graduate or equivalent, required; Associate Degree preferred


  • BLS certified required


  • Medical Assistant certification preferred


  • Certified Navigator- Breast ,CN-BC preferred or completed 2 yrs of  hire
