Discharge Planning Coordinator - Care Navigation- Acute - Per Diem - Days

The Care Navigation department at Toledo Hospital is responsible for the transitions (discharge planning) of adult patients. We currently have 40 employees made up of nurses and social workers who collaborate with each other to cover all areas of the hospital. Our employees work with each patient to establish a safe and effective plan for their transition from the hospital to home care or to a Skilled nursing facility, Inpt Rehab facility or a long-term acute care hospital. We are guided in our role by the Medicare Conditions of Participation. Job Requirements
* Must have Bachelors Degree.
* LSW required.
* Requires knowledge of hospital policy, organization structure and a high degree of discretion secretarial administrative skill.
* Requires specialized secretarial training, including typing, PC word processing. Requires computer spreadsheet skills and database skills.
