The RN Case Manager ensures appropriate utilization of resources, facilities, and services by the review of admission, continued stay, and professional services rendered. Identifies patient discharge needs and works in collaboration with nurses and physicians in managing the patients care needs along the continuum. Establishes and maintains a good rapport and professional relationship with fellow employees, physicians, patients, and other departments. Follows policies and procedures of West Florida Hospital. Participates in the Quality Improvement process and adheres to WFH standards of behavior.
Manages stress appropriately and can make decisions under pressure, handle multiple priorities, work in areas that are confined or crowded. Light to medium work with periods of standing, lifting (50 lbs), stooping, bending, walking, talking, answering telephones, using computer, push/pulling with force and reaching above head and shoulders. Must be able to distinguish colors, hear alarms and blood pressures. Good manual dexterity and clarity of vision. Attendance is essential to this role.
* Associate's Degree (or Diploma) in Nursing is REQUIRED; Bachelor's Degree in Nursing is preferred.
* FL RN license and/or approved multistate license REQUIRED
* A minimum of one (1) to three (3) years of nursing experience REQUIRED
* Case management experience preferred
