Job Description:

As the Functional Family Specialist you will be responsible for assessing client interactions, formulating interventions based on the identified treatment plan, evaluating the progress of treatment goals and providing direct care services. The position will be assigned to the 1st and 6thjudicial district cases.

Responsibilities:

Family Support (Prevention)


  • Understands community resources.

  • Assess child & parent’s level of development and support needs.

  • Mediates conflicts and problem-solves breaking of real or perceived barriers to the child/family’s involvement in the community.

  • Networks with community service representatives to advocate for accessibility of non-traditional resources for families.

  • Facilitates support networks for involved families & youth through a variety of venues including but not limited to family/youth nights, talent shows, holiday parties, informal support groups, etc.

In-home Support


  • Assists in delivery of parent materials and resources in the home setting.

  • Provides a safe learning environment for the parent and child.

  • Supports the family in the implementation of the child development plan.

  • Interfaces with other program staff/Empowerment Board to ensure that the educational components and needs of the family are met.

Care Management Implementation


  • Follows the standards set forth in the Agency Case & Treatment Coordination manual.

  • Coordinates interdisciplinary treatment in line with authorized purchased services.

  • Documents service delivery, including objectives of treatment and intervention strategies.

  • Facilitates community resource planning, case progress evaluation & permanency planning.

  • Coordinates interdisciplinary crisis planning.

  • Participates in discharge planning & documentation.

  • Interfaces with school, medical & psychiatric contacts, referring worker and collaterals.

Education/Experience:

A Bachelor’s degree in psychology, sociology, social work, counseling or related fields with a minimum of two years experience in family therapy or youth services is required.

Juvenile Court Services Officers in the First, Second, and Sixth Judicial Districts may make referrals to Four Oaks Functional Family Therapy. Functional Family Therapy is an evidence-based family intervention program that reduces the possibility of re-off ending crimes.
