Job Description:

Primary Care Provider (APRN)

Community Alliance is looking for a Primary Care Provider to provide the full scope of primary care services, including but not limited to diagnosis, treatment, coordination of care, preventive care, and health maintenance to Community Alliance clients/patients. The Primary Care Provider also functions with the Practice Manager to provide overall clinical leadership and responsibility for monitoring consumer treatment and services provided by the clinic.

Job Duties:


  • Demonstrate leadership, advanced practice skills and teamwork in the delivery of primary care services to individuals in accordance with organizational and program philosophy, goals, policies and practices and within the employee's scope of licensure and training inclusive of:
  • Analyze and interpret consumers' histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses.
  • Diagnose and treat acute and chronic health care problems.
  • Develop treatment plans based on scientific rationale, standards of care, and professional practice guidelines.
  • Recommend diagnostic or therapeutic interventions with attention to safety, cost, invasiveness, adherence, and efficacy.
  • Prescribe medications based on efficacy, safety, and cost as legally authorized.
  • Educate consumers about self-management of acute or chronic illnesses, tailoring instructions to consumers' individual circumstance. Provide consumers with information needed to promote health, reduce risk factors, and prevent disease.

Qualifications:


  • Graduate from an accredited medical or nursing school; Board certified or board eligible in internal medicine, primary care, family practice or other relevant specialty (MD) or family practice (APRN).
  • License in good standing as a Physician (MD or DO) or Advanced Practice Registered Nurse (APRN) in the State of Nebraska.
  • Active Federal DEA License
  • Demonstrated knowledge, clinical skills, and experience providing preventative and primary health care and treatment of adults in an outpatient setting.
  • Demonstrated knowledge, understanding, and empathy for individuals with serious mental illness and complexities of individuals living with serious mental illness and co-morbid physical health conditions.
  • Demonstrated knowledge and skills to work within an integrated care setting and provide person-centered and trauma informed medical care in collaboration with multi-disciplinary and integrated team.
