Job Description:

La Vida Llena LifePlan community is currently seeking a qualified Minimum Data Set (MDS) Coordinator for our 50- bed Healthcare Department that has primarily long-term residents.
The MDS Nurse position requires an understanding of the MDS 3.0 and RAI rules and regulations. The job requires nursing assessment skills and the ability to conceive and create care plans based on a comprehensive resident assessment that is accomplished through resident and family interviews, physical assessment, observation, and input from the Interdisciplinary Team.
Qualifications:
* Registered Nurse, currently licensed in New Mexico
* MDS experience is preferred
* Flexibility, attention to detail, and good computer skills are essential
* Must be an excellent team player
