What You'll Do
The RN, Case Manager is responsible for managing a patient's health needs through the full continuum of care.
The successful candidate will be able to:
* Assess the ongoing needs of the patient and family and make referrals to the appropriate community resources and other health care services
* Provide collaborative, proactive care management that promotes quality and cost-effective health care outcomes
* Work with insurance and third party payers for continued stay authorizations when necessary
* Work to ensure timely and appropriate discharge planning
* Serve as an advocate for patients and their families in regard to decisions affecting plan of care
* Perform utilization review to ensure appropriate use of health care resources
WHat You'll Bring
* Associate's degree in nursing required, BSN preferred
* 5 or more years of bedside nursing experience preferred
* Med-Surge experience preferred
* Must have a passion for excellence
* Must demonstrate excellent written and verbal communication skills
* Professionalism displayed through sound judgement, initiative, and flexibility
* Good interpersonal, communication, team-work, and customer service skills
* Strong organizational, problem-solving and follow-through skills
* Ability to multi-task constantly, react to changes quickly/productively, and tolerate interruptions
* Ability to perform work when no directions are given, and give directions to others
* Remain flexible, organized, professional and able to analyze, make sound decisions under stressful situations
* Demonstrate maturity and accountability
