Work Hours/Shift:

Full Time Days

 

You Will Be Responsible For:

·    Complete nutritional initial, quarterly, annual and significant change reviews on residents according to federal and state guidelines.

·    Review monthly and weekly weights to determine those residents which have had a significant change.

·    Complete nutritional reviews monthly on high risk residents (significant weight loss/ gain, pressure ulcer, hemodialysis, and tube fed).

·    Assess nutritional needs, diet restrictions and current health plans in order to develop and implement dietary care plans and provides nutritional counseling as needed.

·    Attends and participates in care plan conferences, Quality Assurance, and other clinical meetings as directed.

·    Consult with physicians and other health care personnel (Nurse Practitioners, Dialysis Dietitians, etc.) to determine diet restrictions and nutritional needs of residents.

Qualifications

What You Will Need:

·         Bachelor’s degree in related field

·         1 year of long-term care related experience

·         Registered Dietitian

·         Proficient in the use of a computer

·         Typing of 40 wpm

·         Must be 18 years of age or older

 

 

Job Summary:

 

·         Understand and carry out exactly, detailed oral and written instructions

·         Knowledge of good nutrition, diet therapy, and charting procedures

·         Ability to maintain a good working relationship with nursing and other            dietary staff

·         Effectively communicate with others

·         Be familiar with all pertinent regulations affecting the Long-Term Care            dietary service, both State and Federal

·         Ability to read, write, and speak English
