Healthcare Services Manager


Licensed Physical or Occupational Therapist preferred


Position will oversee Outpatient clinics within our retirement communities in Knoxville and Asheville

Responsibilities


Responsible for creating an exceptional Brookdale Healthcare Services customer experience ensuring quality patient care through timely scheduling and delivery with prompt follow up of care needs. Maintains the objectives, goals, and philosophy of healthcare delivery in conjunction with the Brookdale Healthcare Services mission and cornerstones. Partners with agency clinical leadership to monitor efficiency levels within operations assuring prudent financial resource utilization. Ensures efficient and effective communication among community leadership and interdisciplinary Brookdale Healthcare Services team.



  1. Facilitates a consistent service delivery of the Brookdale Healthcare Services continuum including highest quality care for the patient and family.

  2. Actively participates in resident need identification processes including, but not limited to Collaborative Care and the Care 3 Program, and is accountable for completing the referral process including communicating to the patient, the family, the staff and other professionals as appropriate.

  3. Provides follow-up on patient hospitalizations and makes contact with patients returning from the hospital within 24 hours.

  4. Identifies clinical program needs and implements with community leadership and agency clinical leadership.

  5. Participates in consistent meetings with Healthcare Services Director to review customer experience, quality of care delivery, program growth, and leadership skill development.

  6. Support the on-boarding process of Healthcare Services associates.

  7. If applicable, will manage out-patient therapists including, but not limited to on-boarding, coaching, and performance review.

  8. Serves as an educational resource to the community regarding healthcare services. Coordinates available resources for staff and families. Markets BHS to any appropriate audience.

  9. Participates in the continuous quality improvement program as required. Supports effective continuous quality improvement and Outcome Based Quality Improvement (OBQI).

  10. Ensures regulatory compliance of any certified Rehab Agency sites.

  11. Monitors and directs effective utilization for Brookdale Healthcare Services documentation standards and expectations.

  12. Reports on trends and activities using defined reporting processes and assists with action plans to correct, improve, or advance the identified trends and activities.

  13. Works in conjunction with the local Brookdale Healthcare Services agencies to ensure appropriate utilization and clinical outcomes for patients.

  14. Fluent in running all necessary reports to operate the business. Demonstrates the ability to analyze the data and think critically through problems and solve identified issues to achieve the business objectives.

  15. Leads new initiatives and is responsible for the successful implementation of programs. Takes responsibility for learning and managing new programs or initiatives within their area.

Qualifications


Education and Experience
Bachelor’s Degree in therapy related program, graduate of an accredited school of professional nursing, or appropriate related healthcare based degree required. Two years of multi-site and/or multi product line supervisory experience preferred.



Certifications, Licenses, and Other Special Requirements
Current state licensure and/or minimum qualifications preferred for a nurse or therapy position. Travel is required; must have a reliable transportation with current insurance coverage and a current driver’s license.

Knowledge and Skills
Has significant knowledge of an advanced discipline with a working knowledge of related fields. Has significant knowledge of the organization, work environment and process. Has knowledge to direct multiple functions and/or departments with full responsibility for operational results. Has working knowledge of the organization. Basic computer software skills required. Ability to work holidays and weekends as operation demands. Ability to maintain the highest standards of ethical, professional and clinical judgment. Ability to address staff, patients and visitors in a courteous and friendly manner. Must be willing and able to communicate with patients of various levels of functioning and various types of personalities. Willingness to communicate perceived physical and emotional needs of patients to other skilled staff to assist with addressing their care needs.
