Job Description:
Provides outpatient assessment, psychotherapy and psychological management support services primarily to individuals with physical and cognitive disabilities and chronic pain. Conducts psychoeducational support groups for patient and family members. Coordinates planning and service delivery with all transdisciplinary team members while honoring all relevant legal, regulatory and fiscal guidelines. Collaborates with all team members to enhance the transdisciplinary rehabilitation model across the CRH continuum. Requires: California LCSW license.
