Responsibilities
This position will work in the Specialty Clinics and Specialty Pharmacy Hub to provide services to patients and assist patients in getting their medications. The pharmacist's responsibility is to monitor drug therapy lo achieve desired outcomes and to avoid drug toxicity and the dosing of medication based on patient demographics, available treatment guidelines and response. The pharmacist works as an integral part of the patient care team and assists physicians and nurses in providing drug therapy.

Required Qualifications

* Doctor of Pharmacy Degree from an accredited college or university or equivalent experience.

* Must be California Pharmacist License eligible and possess a valid State of California Pharmacist license at the time of hire.

* Completion of a Pharmacy Practice PGYI and/or PGY2 Residency or equivalent experience.

* Thorough understanding and competency with aseptic technique and safe handling of hazardous meds.

* Computer skills that facilitate clinical patient data medication order entry and other basic clinical software programs.

* Demonstrated ability to effectively communicate with and educate patients about their medication therapy.

* Written and oral communication skills to clearly and concisely provide information, explanations and instructions to, and to elicit information from others with varying levels of ability to understand, including individuals who are ill or distressed, or who may not speak English.

* Knowledge of current ambulatory care guidelines (i.e. NCEP, JNC, ADA).

* Demonstrated, ability to identify medication-related problems in patients, communicate them to a patient's physician and offer recommendations.

* Complete working knowledge of, and skill to apply, practices, principles and techniques of patient care related to area of expertise.

* Decision-making skills to independently exercise delegated authority to take action, and to recognize when it is appropriate to seek assistance from others when limits of delegated authority have been reached.

Preferred Qualifications

* Experience with navigating third party reimbursement programs

* BPS or CSP Certification

* Experience precepting and evaluating residents and students

Special Requirements

* The position will include day shifts, evening shifts, night shifts and rotating weekend shifts.

* Basic CPR certification required upon employment.

* Must have good personal hygiene and dress in a professional manner consistent with the Departmental professional behavior policy.

* Abide by Departmental attendance standards.

* Special protective equipment must be used for required preparation of hazardous substances and spill cleanup.

* This position may be subject to a criminal background investigation, drug screen, Live Scan fingerprinting, medical evaluation clearance, and functional capacity assessment.
