As a Home Health RN you will:
• Provide direct care to the patients, including treatments and medications
• Develop, follow, and coordinate each patient's comprehensive assessment and plan of care
• Notify physician, when necessary, of changes in patient's condition
• Complete all necessary documentation according to regulations and company policies and procedures

Qualifications and Skills:
• Current PA RN license in good standing
• Prefer one year home health experience
• Self-directed with excellent observational ability and effective communication skills
• Ability to complete OASIS Assessments both timely and accurately
• Valid driver's license with current auto insurance
