Job Description:
Assist Outreach Workers in the field on service delivery and provide consultation when working with homeless clients as needed. Assist with training to Outreach Workers on HMIS data entry. Assist with completion of monthly and annual HMIS data reports. Maintain clear, accurate and concise consumer records, in a timely manner. Assist with program design and implementation. Provide professional linkage/consultation with peers, professionals of other disciplines and outside entities. Perform daily outreach, engagement and linkage to homeless individuals. Act as a team liaison between supervisor and outreach workers. Provide other duties as assigned.
