The Staff Nurse will be responsible for:
Providing nursing support and care to patients in the Watkins Cardiology clinic of the Ambulatory Division.
Providing telephone and/or Patient Gateway triage for patients with a variety of cardiovascular conditions.
Collaboration with Watkins Cardiology Access Center, Clinic, and Arrhythmia Services Ambulatory RN for routing message management for incoming patient calls.
Communication with patients via electronic health information exchange Patient Gateway/MyChart, phone, fax, mail, and email as necessary.
Communication with providers using SBAR methodology: Situation, Background, Assessment, and Recommendation.
Gathering patient data and follow up on requests/review consults with MD/NP to develop care plan.
Education of patients and families regarding cardiovascular disease and management in person, telephonically, and virtually as appropriate for the ambulatory setting.
Completing post-hospital discharge phone calls including medication reconciliation.
Seeking to maximize communication efficiency with MD, LIP for patient-centered care delivery.
Performing nurse only visits as indicated.
Setting up consults with specialty providers, following up on consult recommendations, and communicating results to patients and providers.
Performing all other duties as required or assigned within the scope of the position.
Working collaboratively to provide excellent care to patients and families in the safest environment.
Qualifications
To qualify you must have:
Graduate of an approved school of nursing with current registration in Massachusetts. Bachelor of Science Degree in Nursing is required.
Minimum of 2 years recent (within 1 year) experience with cardiac-focused patient care delivery as an RN required.
Previous clinical nursing experience in an ambulatory setting with telephone triage and telephone-based clinical assessment techniques required.
Previous experience providing ambulatory nursing care to a cardiovascular patient population required.
Experience providing in-person triage and direct patient care for acute and chronic conditions as appropriate for a cardiology setting strongly preferred.
Case Management experience preferred.
Nursing certification in Ambulatory Care or ambulatory specialty area preferred.
Experience with team-based care preferred.
