Located in the heart of World Golf Village, Westminster St. Augustine is currently accepting applications for the position of Assistant Director of Nursing to work on a full-time basis. The Assistant Director of Nursing will oversee and direct nursing care for the residents, and to supervise the daily nursing activities on assigned units of the Health Center. At the discretion of Administration, the Assistant Director of Nursing may be required to act as interim Director of Nursing in the event of their absence or vacant position. This position will also serve as the MDS Coordinator and is responsible for the coordination and completion of Minimum Data Set (MDS), Admission Assessment, and Resident Care Plans.

ESSENTIAL JOB FUNCTIONS:

The Assistant Director of Nursing must comply with all applicable rules, policies, standards and guidelines related to employment with Westminster Services and its communities, including:


  1. Direct and supervise, on assigned unit, the daily functions of the nursing staff in accordance with current rules, regulations, and guidelines that govern the Skilled Nursing Facility.

  2. Ensure that all nursing personnel assigned to assigned care area comply with the written policies and procedures established by this facility.

  3. Participate with updating and reviewing care plans that identify goals, problems, approaches, and revisions based on nursing needs.

  4. Administer professional nursing services when needed commensurate with the scope of practice.

  5. Meet with assigned nursing staff to discuss planning the shift’s services, programs, and activities and monitor duties are completed as directed.

  6. Collaborate with other members of the interdisciplinary team to ensure that the resident’s total regimen of care is maintained.

  7. Manage quality and accuracy of the EHR for all residents and staff under your oversight

  8. Manage and improve unit based performance metrics that are reported by the facility (ie. falls, FAPU, QMs, Re-hospitalizations, etc.).

  9. Oversee the quality of resident admissions, transfers and discharges.

  10. Participate in the orientation of new residents/family members to the unit.

  11. Complete daily clinical and environmental rounds to evaluate resident care and report significant events.

  12. Address resident/family issues in a timely manner. Report significant issues to the Director of Nursing.

  13. Communicate with health care practitioner to review treatment plans, complete orders, progress notes, etc., in accordance with established policies.

  14. Oversee the completion of accident/incident reports involving residents. Report to IDT.

  15. Complete Recapitulations each month for physician signature. Report all discrepancies to DON.

  16. Ensure that an adequate supply of floor stock medications, supplies, and equipment are available to meet resident care needs.

  17. Conduct employee performance evaluations.

  18. Make recommendations to the DON concerning employee discipline, dismissal and/or transfers.

  19. Document MDS in the Electronic Charting System (ECS) in an accurate and timely manner.

  20. Complete daily MDS assessments (admission, quarterly, etc.) through Resident Interview Assessments on schedule.

  21. Update resident care plans (falls, wounds, infections, etc) on a daily basis.

  22. Maintain the PPS and OBRA schedule.

  23. Monitor all members of the care plan team to ensure accuracy and completion of the MDS.

  24. Manage and schedule weekly care plan meetings. Meet with family members and interdisciplinary teams during the care plan meetings.

  25. Prepare and transmit MDS assessments to the state on a weekly basis.

Required Knowledge, Skills and Abilities:

  • Must be able to work with ill, disabled, elderly, emotionally upset, and potentially hostile people within the facility.

  • Must be able to speak, write and understand English in a manner that is sufficient for effective communication with supervisors, employees, residents, and families.

 

ESSENTIAL QUALIFICATIONS:

 Education: Must possess a current, unencumbered, active license to practice as an RN in the State of Florida. Registered Nurse Assessment Coordinator (RNAC) Certification preferred. Must be CPR Certified. Must possess physical assessment skills and be able to function independently throughout the Health Center. Must work methodically with record keeping, time frames, and documentation. Must have one (1) year experience completing MDS.

Experience: Must have a minimum of 3 years of long-term care experience as a staff nurse; at least 6 months experience as a nurse leader (direct or in-direct care). Prefer supervisory leadership experience.

 Physical and Environmental Requirements: Balancing, Kneeling, Reaching, Crouching, Pulling, Standing, Manual Dexterity, Pushing, Stooping, Fingering, Grasping, Utilizing a computer, Lifting up to 40 pounds

Sensory and communicative activities with or without prosthetics of this position are: Feeling, Smelling, Hearing, Speaking, Seeing, Typing

An individual in this position will be exposed to: Inside/outside environmental conditions, bloodborne pathogens, respiratory infections

Must be able to cope with the mental and emotional stress of this position.
