Responsibilities

The After Hours Triage Visit Registered Nurse plans, organizes and directs hospice care and is experienced in nursing with emphasis on education and communication. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities as necessary.

1. Completes an evaluation and / or a complete initial assessment of patient / family / caregiver to determine hospice needs.

2. Provides professional nursing care by utilizing all elements of the nursing process.

3. Based on health assessment data, develops, implements and coordinates a plan of care that establishes goals and incorporates palliative nursing actions. Includes the patient / family / caregiver and Interdisciplinary Team (IDT) members in the planning process.
4. Assesses and evaluates patient's status by initiating and documenting a plan of care, regularly re-evaluating patient / family / caregiver needs, and revising the plan of care as necessary.
5. Initiates appropriate preventive, palliative and rehabilitative nursing procedures. Administers medications and treatments as prescribed by a physician. Evaluates patient's response to treatments / medications.
6. Facilitates patient's activities of daily living and efforts toward self-sufficiency, quality of life and optimal comfort care. Provides and maintains a safe environment for the patient.
7. Ensures that DME / medical supplies and other necessary items and services are available.

8. Assists the patient / family / caregiver and IDT members in providing continuity of care.

9. Provides hospice services directly to patients in the patient's home or other setting, including but not limited to a nursing facility, assisted living facility, residential care facility, or adult foster home, which is considered to be the patient's home.

10. Facilitates professional communications and works effectively between Signature Hospice employees / volunteers, patients / families / caregivers, physicians, vendors, and other departments and professionals as appropriate.
11. Completes, maintains and submits / synchronizes accurate and relevant clinical notes and electronic documents regarding patient's condition and care given. Records pain / symptom management changes / outcomes as appropriate.
12. Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains / receives physicians' orders as required.
13. Teaches the patient / family / caregiver self-care techniques as appropriate. Recognizes and utilizes opportunities for health counseling with patients / families / caregivers to meet plan of care goals. Works in collaboration with IDT Members.

14. Fulfills the obligation of requested and / or accepted assignments including but not limited to being Case Manager, performing admissions and / or supervising ancillary personnel.
15. Consistently represents Signature Hospice with integrity and professionalism to all internal and external customers.
16. Ensures strict adherence to Signature Hospice safety and infection control practices. Reports all incidents and accidents.
17. Actively participates in IDG meetings, staff meetings, department meetings, in-services, QAPI activities, Safety Committee and other related activities.
18. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
 

Qualifications/Skillsets

1. Graduate of an accredited school of nursing. Bachelor's degree, with one (1) to two (2) years of recent experience in hospice or home care and one (1) to two (2) years of recent acute care experience in an institutional setting preferred.2. Current professional licensure in State and valid driver's license and automobile insurance in accordance with State and / or organization requirements.
3. CPR certification required.
4. Management experience not required. Responsible for supervising hospice CNA/HHA and LPN/LVN.
5. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
6. Must be capable of performing the Essential Job Functions / Responsibilities of this job, with or without reasonable accommodations.
7. Possess basic computer skills and knowledge of office technology - e.g. email and internet services, facsimile machines, telecommunication devices, etc.
