Clinical Coordinator at Heritage Communities in Omaha, Nebraska

Job Description:

This position is to assist in achievement of the vision of Heritage OnCare Home Health, by performing the duties as assigned, in a results-oriented, cost effective, and customer focused manner. This position will coordinate all referrals received by the agency, complete necessary screening of referrals, perform insurance authorization procedures as needed, update insurance authorizations as needed, work with Coding Specialist as appropriate, as well as process all resumptions and recertification, assign admissions, and consult with admitting clinicians on specifics of client care needs as applicable.

RESPONSIBILITIES:

1. Knows client legal rights, respects client rights at all times, and reports any failure of any other person to comply with client rights including client abuse, neglect, or disrespect.


2. Maintains confidentiality with regard to all client's personal, financial, and health information and company business including following HIPAA guidelines. Does not share with client's company issues, concerns, or problems.


3. Accommodates client, referral source, and referral needs and requests in a timely and positive manner.


4. Maintains a productive and harmonious working relationship with all other staff within the company as well as clients, families and referral sources.

• High school Diploma or GED


• Graduate of an A.D.N or Nursing Diploma Program


• Licensed as a Registered Nurse or Licensed Practical Nurse


• Home Health background strongly preferred
