The Health Care Quality and Affordability team is made up of about 1,000 doctors, nurses, pharmacists, strategists, industry experts and innovators with a single vision – to transform the delivery of health care. Every day, we work together to improve the quality of patient care for our members while lowering healthcare costs to ensure all Californians have access to the care they deserve.

We are continuing to expand our network of providers to ensure state-wide access to care. We evaluate analytic and digital health tools that simplify care delivery while improving member health status, healthcare quality and affordability as well as member and provider experience.

 

We provide high-touch services to members from experienced clinical teams. You will also find us collaborating with our federal and state legislative officials to help shape the policy to advance our Blue Shield mission. 

 

The Community Health Program helps meet multiple goals for Blue Shield of California and is aligned with our collective mission, which will impact social determinants and lead us towards health equity. As part of the effort to transform healthcare, it is critical to promote community-clinical partnerships and services aimed at managing and improving health at the community level. Care delivery and community-based organizations and agencies need to partner with each other to develop cooperative strategies for the design, implementation, and accountability of services aimed at improving individual and population health. These coordination and communication needs for patients may be more acute when patients require services beyond the traditional health care system, such as social and community services, for managing their condition. 

 

The Program Manager, Community Health is responsible for managing and coordinating community health program pilots that will work to impact and improve the lives of individuals. In collaboration with the Senior Manager, Community Health, the Program Manager participates in strategic, collaborative partnerships with providers, hospitals, and community stakeholders to achieve goals. The Program Manager is also accountable for the management of assessments, planning, evaluation and reporting of community health program initiatives focused on improving the lives of entire communities – members and non-members alike. 

 

The Program Manager, Community Health, will be responsible for the development and launch through successful transfer of one or more multi-year external facing programs within a business unit. Program Manager, Community Health will support business strategies through an integrated portfolio of projects or initiatives and may have responsibility for a larger enterprise/regional external facing program. 

 

Program Manager, Community Health supports the development and delivery of a range of initiatives and resources that enable healthcare and community to improve community health by addressing essential resource needs required to support health equity. Program Manager, Community Health will develop and apply their knowledge of essential resource integration with both healthcare and community stakeholders by executing a range of programs/services and demonstrations of learnings. Program Manager, Community Health will also support the development of learning and implementation guidance that stems from their work. The Program Manager, Community Health, will report to the Senior Manager, Community Health, and be based in Southern California.  

 

Responsibilities


  • Drive successful delivery of assigned programs through excellent project management, relationship management, and consulting / presentation / facilitation responsibilities as assigned. Programs may focus on demonstration or spread and deliver learning and/or consulting support.

  • Support successful design and delivery of programs by executing on research and analysis, and quality improvement as needed and engaging internal / external experts to ensure high impact strategies, structures, and inclusion of best knowledge available. 

  • Support strong communications and relationship management with partners, with a special emphasis on identifying, testing, and implementing new models and practices from the most innovative health care and community partners in our network, as well as experts. 

  • Manage the launch of programs that enable partners to identify, test, and implement practices that address their community’s essential resource needs in the service of improved health equity. Organizes meetings of key stakeholders and community leaders while documenting progress and holding key members accountable for work products. 

  • Research, design and recommend opportunities to enhance pilot efforts through scaling best practices, technology platforms and ideas generated by impacted communities. 

  • Facilitates community engagement strategies and partnerships towards health innovation and best practice identification in community health improvement. 

  • Assist project and program leads in the administration of multiple projects, helping ensure that required tasks are completed, risks to project deliverables are reported and mitigated, and open issues are resolved with minimal disruption to project timelines.  

  • Creates and delivers presentations, reports, and profiles to share successes, opportunities and stories from the community health pilots. 

  • Create processes and procedures for cross-initiative tracking. 

  • Aids in the development of the community health strategic plan.

  • Other duties as assigned. 

 








Knowledge, Experience and Education 


  • A Bachelor’s degree is required; advanced degree (MBA, MHA, MPA, or equivalent) preferred. 

  • 5 or more years of experience working in a community or public health role. 

  • Working operational knowledge of the key synergies necessary among practices, payers, community, and policy leaders to better align health care transformation efforts. 

  • Understanding of social determinants of health, public health concepts, community engagement, ACEs, Trauma informed care and health equity highly preferable.  

  • Demonstrated ability to think strategically and operationalize goals and priorities. 

  • Cross-cultural agility, the ability to relate to a wide variety of diverse audiences with strong emotional intelligence and empathy. 

  • Highly talented leader, with the ability and desire to think creatively and innovatively. 

  • Possess a performance and results driven project management style. 

  • Strong interpersonal skills, including communications, problem solving and negotiating skills. 

  • Able to sustain progress within dynamic and potentially ambiguous environments. 

  • The ability to adapt to change and can shift gear comfortably and quickly.  

  • Able to identify the key insights / practices arising from programmatic work that others (internally and externally) can benefit from; ensures these insights / practices are well-codified.  

  • Familiarity and comfort using technology to manage program performance. 

  • Proficient with computer programs such as Microsoft Excel, Outlook, Word, and PowerPoint. 

  • Effective communication skills (written, oral) and some experience with and interest in facilitation; able to create presentations / content / curriculum for a diverse audience (front line staff, community stakeholders, leadership in healthcare, and community-based organizations (CBOs)), and engage external partners in varied ways on how to test and implement the concepts and practices contained within. 

  • Proficient project management skills, with demonstrated ability to manage self, team members and stakeholders to identify obstacles and ways to address and meet deadlines.
