As a core member of the Care delivery team, the Clinical Nurse practices as the patients nurse and collaborates
with the team to facilitate the coordination of patient care at the unit level. Through utilization of the nursing process ,
the clinical nurse diagnoses and treats patient and family responses to actual or potential health problems. The
Clinical Nurse is accountable to the patient, family, peers, and Manager.
Patient and family centered care (PFCC) at YNHH is demonstrated by working with patients and their families based

30 1. Nursing Process
The Clinical Nurse I (CN I) utilizes the nursing process of assessment, planning, intervention andevaluation while caring for our patients maintaining confidentiality and dignity. The Clinical Nurseprovides safe, effective evidenced based care tailored to the individual needs of the patient and family .As a core member of the care delivery team, the Clinical Nurse partners with the team to facilitate thecoordination of patient care at the unit level utilizing the principles of patient/family centered care.

The clinical nurse uses a 5 step process to deliver high quality comprehensive and individualized careincorporating:
1. Patient assessment
2. Problem identification
3. Establishment of desired outcomes
4. Implementation of interventions
5. Evaluation of care with necessary revisions.

Qualifications
EDUCATION (number of years and type required to perform the position duties):
Graduation from an accredited School of Nursing., Licensed and a currently registered nurse in the State ofConnecticut., All newly hired Registered Nurses musthave current BCLS certification or must obtain within one month of hire.
BSN preferred
EXPERIENCE (number of years and type required to meet an acceptable level of performance):
Clinical Nurse is the basic level for new graduates and all registered nurses ., All RNs are expected to advance totheir optimal level of functioning through promotion.
SPECIAL SKILLS:
Excellent communications, interpersonal, coordinative, and organizational skills.
ACCOUNTABILITY (how this position is held accountable for such as goals achievement, budget adherence, orother areas of accountability):
Provides nursing care to an individual or group of patients; recognizes changes in patients condition and reportssuch changes to Physician and/or Patient Services Manager., Utilizes and manages material resources in acost effective manner.

SPECIAL WORKING CONDITIONS AND PHYSICAL DEMAND:
Requires prolonged standing; walking; use of sight, hearing, and touch; moving/lifting patients of varying weights.
