As an acute care Medical Social Worker, you will coordinate and provide clinical social work and/or discharge planning services (in conjunction with the Case Management Team) to patients and families.

  You will complete comprehensive assessments, which take into account the patient's current and prospective functional capacities, resources and support systems.

  
  


• 
  

  You will develop and implement patient-centered treatment goals and plans utilizing appropriate therapeutic interventions.

  
  


• 
  

  You will formulate appropriate and realistic patient-centered discharge plans including the arrangement of any necessary follow-up services such as durable medical equipment, home health and/or placement.

  
  


• 
  

  You will advocate in support of the patient's right to self-determination.

  
  


• 
  

  You will educate and/or consult services to patients, families, visitors, physicians, Georgetown Hospital staff and the community.

Licenses and Certifications

  Current Texas Social Work licensure, acceptable licensures include Licensed Master Social Worker (LMSW), Licensed Master Social Worker - Advanced Practice (LMSW-AP) and Licensed Master Social Worker - Advanced Clinical Practitioner (LMSW-ACP)

  Current Basic Life Support (BLS) certification required as per St. David's Healthcare policy
