Chandler Hall offers a wide spectrum of services for individuals at all stages of the aging process including: Community Living, Personal Care, Residential Hospice, Home Hospice, Independent Living, Respite Care, and Skilled Nursing Care. Chandler Hall currently has an opportunity for a Full time Social Worker in our 52 bed Skilled Nursing Home.
The Social Worker will assist in fostering the climate, enhancing the quality of life; emphasize assisting the resident cope, retain their individuality, independence and choice; and ensuring compliance with Federal and State regulations and Chandler Hall standards.
Job Responsibilities:
Maintains the confidentiality of all resident care information and treats all residents fairly and with kindness, dignity and respect.
Attends, participates, and leads scheduled programs, trainings and activities.
Learns continuously by attending all in-services, Relias trainings, attends seminars and various meetings as applicable to always remain in compliance with best practice State and Federal regulation.
Completes all assessments on time and attends all care conferences for designated residents.
Enable residents to see causes, connections and implications, create sound plans; solve problems effectively; set priorities and to make decisions for the good of the whole based on analysis, experience and judgement.
Maintains family involvement with residents by facilitating and encouraging this involvement, and notifying families of resident issues and concerns.
Completes insurance updates for residents with the most current information and gathered information from the Interdisciplinary team.
Ability to navigate the MDS, care plans and point of care in order to input assessments and to investigate about a residents care.
Proactively anticipate and identify potential problems, issues and opportunities by monitoring the residents adjustment to facility living, health, treatment plans and goals.
Participates in facility survey (inspections) made by authorized government agencies.
Assist, develop and revise the departmental policies, procedures, forms applicable to social worker’s functions.
Job Requirements: Bachelor's Degree in Social Work or related field. 3-5 years Gerontology Experience in a skilled facility preferred.
