Job Description:

We Are Hiring: Casa Scalabrini, part of Ascension Living, is looking for a caring and compassionate Social Services Specialist to join the team! This gorgeous skilled nursing and rehabilitation community is ideally located close to I-294 in Northlake, and offers high level nursing care, memory care and respite care.

  • Full-time
  • Monday thru Friday, 8A to 4:30P with occasional weekends

What You Will Do: Assist clients by assessing their social services needs, solving problems, setting goals and obtaining required services.

  • Conduct initial intake procedures. Assess needs and eligibility of potential clients.
  • Participate in the initial assessment process, service plan of care development, and ongoing monitoring of each client's care.
  • Obtain extensive social history from client and/or family member upon admission to program.
  • Resolve client problems by clarifying the issue, determining the cause; selecting and explaining the best solution; expediting correction or adjustment; and following up to ensure resolution.
What You Will Need: Licenses / Certification / Registration:

  • Social Worker preferred.
Education:

  • High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required.
Work Experience:

  • 3 years of experience preferred.
