Job Description:

Westminster Oaks is seeking a full-time Nurse Case Manager/Liaison. The successful candidate will be responsible for performance of clinical assessments for potential and current Westminster Services Health Center residents. Negotiation for residents who are clients of managed care organizations. Liaison for community relations, discharge planners and physicians. Provide support for all Marketing efforts. Maximize cost effectiveness across the entire continuum of care; improve customer satisfaction and enhance quality of care.  Must be a licensed RN or LPN in the State of Florida. Certification in Case Management or Geriatrics desired. Must be CPR certified. Must possess at least two years experience in long term care, including working knowledge of Medicare and Managed Care protocols and one year in post acute rehabilitation. Must have knowledge of case management purpose and process, with demonstrated clinical assessment skills.
