As a Home Health Coordinator (HHC) you will support our Dallas Fort Worth offices - LVN preferred
Provide sales support for our Account Executives within senior living communities (independent living, assisted living, memory care)
Initiate, build, and progress relationships within the senior living market and senior living communities (independent living, assisted living, memory care)
Assist our Account Executives in being a valuable resource for senior living communities through educational presentations, wellness clinics, and other services based on community needs
Facilitate the home health referral process
Coordinate information to residents, family members, physicians, senior living community/SNF/hospital staff and any other involved entities related to the continuing care of residents in their homes
Assist in providing senior living management with adequate information on care plan prior to the resident's order for service and equipment, while communicating with the home health agency
Provide home health agency with complete referral information to ensure timely, appropriate care, and to enhance relationships between the agency, residents, families, physicians, and referral sources
Participate in senior living community care coordination meetings
Facilitate and be a valuable resource for residents, family members, and staff for monthly support groups
Requirements of Qualified Candidates
One (1) year of home health experience preferred
One (1) year of senior living experience (independent living, assisted living, memory care) preferred
RN or LVN preferred
Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and in good working order
