ABOUT THE JOB
The Jewish board is currently seeking a transition coordinator to work at our Westchester location. As part of a multi-disciplinary team they will be responsible for all discharge duties including: maintaining case documentation, identifying and facilitating family and resident connection to community based agencies and services.
The transition coordinator will also provide after care to families and residents by conducting initial home assessment and complete discharge needs assessment as well as fostering connections with discharge resources to benefit the participants and the organization.
TO QUALIFY
The Jewish Board is currently looking for talented people of all cultures, religions, races, andgender expressions with the following qualifications:
BA/S with a minimum of 1 years' experience in the mental health field required. OR MA/S with a minimum of 1 year of experience in a residential setting strongly preferred
Driver's license required
Bi-Lingual a plus
