Job Description:

Job Qualifications: Graduation from an accredited nursing program is required. Minimum of six months to one year experience in psychiatric or equivalent mental health related nursing experience is preferred. Must have the ability to work cooperatively with all staff and physicians and function as a valuable member of the treatment team. Licensed Practical Nurse (LPN) in Indiana.

Summary of Job: Functions as staff LPN utilizing the nursing process with performing comprehensive psychiatric /mental health assessments. Evaluates mental status and refers clients to other treatment providers when necessary. Assumes primary responsibility and accountability for ones nursing practices as defined in the Indiana Code of Nursing Practice, as it pertains to a licensed practical nurse and functions under the supervision of an RN or physician.

Essential Duties and Responsibilities:

Supervisory Responsibilities for: This job title does not supervisor others.

Performs comprehensive psychiatric /mental health assessments, evaluates mental status and refers to other treatment providers when necessary.

Monitors changes in health status, including but not limited to vital signs, weights, nutritional status and disease states.

Provides assigned residential nursing services as directed.

Administers and monitors medications as ordered. Monitors medication effectiveness, side effects and compliance. May perform assessments for any side effects/EPS.

Addresses medication issues with consumer when necessary. Provides proper education and guidance as necessary.

Provides healthcare education to clients and/or families or staff as assigned.

Works in collaboration with other members of the treatment team and in collaboration with external health care providers as necessary.

Regular and Reliable attendance is required.

Key Performance Indicators

Meets billable hour productivity standards.

Completes clinical documentation within organization's policy standards.

Maintains all mandatory trainings up to date.
