Job Summary: Clinical Pharmacists are essential members of the healthcare team and work collaboratively with the team(s) to ensure safe and effective medication therapy to improve patient outcomes. Under the direction of the Clinical Manager, the Clinical Pharmacist I works in a decentralized role in the provision of direct patient care.

Essential Responsibilities:


  1. Practices pharmacy in accordance with laws, rules and regulations of the Massachusetts Board of Registration in Pharmacy, Department of Public Health, U.S. Food and Drug Administration, and Drug Enforcement Administration and in accordance with established departmental policies and procedures. Maintains credentials in accordance with the Massachusetts Board of Registration in Pharmacy.

  2. Works collaboratively with the healthcare team through participation on interdisciplinary rounds and through consultation to make evidence-based recommendations on the initiation, discontinuation, and monitoring of medications. Serves as a drug information resource to other healthcare professionals, patients and family members. Provides timely and evidence-base responses.

  3. Reviews and verifies medication orders with consideration of the patient's: diagnosis, weight/body surface area, age, allergies, organ function, laboratory indices, medication duplication, drug interactions, and other patient-specific factors. Clarifies orders deemed to be inappropriate, incomplete, or otherwise unclear with the prescriber in a timely manner.

  4. Documents pertinent information, interventions and/or discussion/s with the healthcare team to ensure continuity of care with other pharmacy staff. Communicates applicable information to subsequent shifts to ensure continuity of care.

  5. Participates in therapeutic drug monitoring programs. Reviews related data and makes recommendations to optimize/modify therapy as determined by patient condition, laboratory results, and/or associated clinical guidelines/protocols.

  6. Demonstrates competency for clinical pharmacy practice including determining appropriate drug therapy for patients, prevention/resolution of drug related problems, and dispensing and compounding skills consistent with good pharmaceutical practice.

  7. Participates in activities/experiences so as to master knowledge of institution specific medication policies/procedures/guidelines and to incorporate within medication management practices.

  8. Participates in departmental quality improvement activities, process improvement, quality assurance, formulary management, and/or restricted medication programs. Attends department committees and/or shared governance activities as directed by manager.

  9. Serves as a co-preceptor/preceptor and/or mentor to pharmacy students in coordination with the direct manager. Works to develop proficiency in the four preceptor roles and build education/mentorship skills. Works collaboratively with all pharmacy teams and team members to ensure pharmacy services support the work of the heath care team and optimize patient outcomes

  10. Maintains a personal portfolio of work activities and projects which support their performance of required job duties. Portfolios will be reviewed bi-annually at minimum with the respective Clinical Manager.

Required Qualifications:

  1. Bachelor's degree in Pharmacy required. Doctoral degree in Pharmacy preferred.

  2. License Massachusetts Pharmacist required., or immediate eligibility

  3. 0-1 years related work experience required.

  4. Completion of PGY1 accredited pharmacy residency or 3+ years of Clinical Pharmacy experience in hospital setting.

  5. Certification in area of clinical specialty (Certified Specialty Pharmacist (CSP), Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), etc.). If not board certified at time of hire, must be certified within 18 months of hire.

  6. Experience with computer systems required, including web based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access.

  7. Team Work: Ability to work collaboratively in small teams to improve the operations of immediate work group by offering ideas, identifying issues, and respecting team members.


  8. Customer Service: Ability to provide a high level of customer service and staff training to meet customer service standards and expectations for the assigned unit(s). Resolves service issues in the assigned unit(s) in a timely and respectful manner.
