Job Description:

The Care Coordination Clinical Lead position leads the clinical aspect of the department programs to include, but not limited to, Chronic Care Management, Transitional Care Management, Remote Patient Monitoring, and the Home Visit Program while working in conjunction with associated community partners to identify, monitor, and manage high risk patients.
