Job Description:

Coast Caregiver Resource Center (CCRC) is a program of Cottage Rehabilitation Hospital, which provides families and other unpaid caregivers with meaningful information, referral, consultation, and other support, enabling them to provide competent care to their loved ones at home, while managing the stress that often accompanies care giving.

We are currently in need of a part-time (20 hrs/wk) Family Consultant who will develop Caregiver Action Plans, provide caregiver consultation, and coordinate respite and counseling services with contracted providers. Position also includes facilitation of psycho-educational and/or support groups and caregiver education. The Family Consultant also represents the agency through public speaking and community outreach; data collection, and provides reports in a timely manner. Close attention to detail, familiarity with community resources and compassion for caregiving families is required. Familiarity with cognitive impairments strongly desired.

Requires Bachelor's degree in Social Work, Psychology, Gerontology or related field and 5+ years' experience with caregiving families and/or dependent adults. Master's degree in social work or related field preferred. General MS Office skills, and valid California Driver License (own transportation). Bilingual in Spanish preferred. This is a grant funded position based in San Luis Obispo, but you will be working remotely.
