Job Summary


Reviews patient records in a timely manner to identify an appropriate selection of codes to accurately reflect the reason for admission, extent of care received, and level of severity of illness. Insures that all data elements required for federal and state reporting are collected and included in the patient?s demographic record. Codes, sequences and abstracts diagnoses and procedures for all outpatient records (outpatient diagnostics and emergency) in accordance with ICD-10-CM, CPT, and Coding Clinic guidelines.


Minimum Job Requirements



  • Completion of an accredited Coding Education program.



  • High school education.


Essential Duties and Responsibilities



  • Assigns ICD-10-CM and CPT-4 diagnosis and procedure codes, using the 3M Encoder, for all outpatient and emergency room records; maintains a 95% accuracy rate when coding and sequencing medical (patient) records.



  • Assists medical staff and hospital with proper nomenclature of medical terminology and proper coding and sequencing of ICD-10-CM and CPT codes.



  • Complies with established coding productivity standards as outlined in department policies and procedures, and seeks supervisor assistance when such standards cannot be met.



  • Maintains and submits to supervisor on a daily basis required productivity logs for all assigned tasks.



  • Performs patient record coding using the Cerner system 100% of the time when patient records have been scanned and indexed into the Cerner system.



  • Reviews unbilled accounts on a daily basis to identify those pending and maintains the work current and within 4 days of discharge and assigned financial goal.


Knowledge/Skills/Abilities



  • Ability to communicate effectively in English both verbally and in writing.



  • Ability to relate cooperatively and constructively with customers and co-workers.



  • Ability to type 30 words per minute preferred.



  • Basic knowledge in use of computers and printers, and/or ability to learn appropriate software application(s).



  • Knowledge of Anatomy, Physiology, and medical terminology.



  • Must be able to maintain confidentiality of sensitive information.



  • Must be proficient in alphabetical and numerical filing systems.



  • Requires good organizational skills and must be adaptable to frequent changes in assignments.
