Job Description Summary:To competently interpret the patients history and baseline level of function and then to complete an assessment appropriate to the patients areas of deficits. Based on problems identified, the physical therapist establishes and carries out a plan of care to include patient treatment and reassessment, patient/family instruction, discharge planning, documentation in the medical record and communication with physicians and other medical staff. This position helps patients achieve maximum functional outcomes. Acts as a clinical instructor for students and actively participates in department functions.
Minimum Qualifications:Bachelor's Degree: Physical Therapy (Required)CPR - Cardiopulmonary Resuscitation - American Red Cross, LIC - Licensed in Field - State of Ohio
BS in Physical Therapy. Licensed in the state of Ohio. CPR certification that is recognized by the American Heart Association.
Basic computer skills.
KIND and LENGTH of EXPERIENCE
Entry-level new graduate, if facility has at least one veteran Physical Therapist to mentor, otherwise, at least one year of clinical experience.
