Responsibilities

Oversees and ensures smooth day to day operations of the Continuous Care Program. Manages Continuous Care services to Hospice patients and their families. Ensures VNSNY has optimum quality and financial outcomes in the Continuous Care Program. Assures documentation of the patient's eligibility for Hospice Continuous Care significant events, plan of care, and other care related issues. Provides regular management reports. Provides back up managerial support to other various Hospice managers during vacancies, vacations etc. Works under general supervision. Licensure: License and current registration to practice as a Registered Professional Nurse in New York State required. EDUCATION: Bachelor's or Master's Degree in Nursing, or enrollment in accredited program within first year and completion of Bachelor's degree in Nursing within six years after accepting position. EXPERIENCE: Minimum three years of nursing experience in a hospice or homecare setting required; management experience preferred. Effective computer and data entry skills required including the ability to use Microsoft Office (Excel, Word, Access, and Power Point). Proficient verbal, written, customer service and interpersonal skills, required. Strong Problem Solving skills, with the desire to initiate and improve work processes. Ability to pay attention to detail, excellent follow-up and follow through skills required. Skilled at handling multiple tasks, in a fast paced setting required. Prior medical billing and scheduling experience preferred.
