The Case Manager is responsible for:
Conduct thorough needs assessments of children and their sponsor families upon case referral. Utilize social work best practices for thorough assessments of child’s progress and services provided.
Perform ongoing case management with a focus on screening and making appropriate referrals to address child’s needs in the following domains: placement issues, safety issues, education, health, mental health, legal/immigration proceedings, substance abuse, independent living, criminal/juvenile justice, and child enrichment activities.
Assures that each child has a complete, comprehensive and realistic Individual Service Plan in accordance with the child’s needs as determined by the Individualized Mental Health Needs Assessment
Provide crisis intervention services directly, as needed, and connect family to services to ensure minor’s safety and well-being in a crisis situation.
Submit detailed and thorough documentation in compliance with funder policies and procedures.
Maintain regular and timely communication with supervisor and funder staff around any issues concerning the child’s safety, stability, or well-being in the placement. Elevates issues or concerns to the Family Reunification Program Manager, Managing Attorney, and/or Director of Esperanza Center.
Attends all mandatory training including the following topics: motivational interviewing, trauma informed
care, behavior management, cultural sensitivity, Catholic Charities policy on sexual abuse and sexual harassment, and mental health and child welfare best practices. Additional training will include policies and procedures, information regarding immigration issues and other matters, including issues specific to unaccompanied children and their families, as well as opportunities to attend trainings on best practices for identifying and serving victims of human trafficking.
This position requires:
Bachelor’s degree in Social Work, or other relevant degree required.
Bilingual (Spanish/English) required.
3-5 years of case management experience, preferably with child/family welfare programs
Knowledge of community resources and ability to connect families to community services
Experience conducting home visits preferred.
Ability to travel to home visits and other service agencies to serve the client. If driving on Agency business, must be at least 21 years of age and have valid driver’s license issued by the state of residence, with no more than 3 points.
Knowledge of and experience working with refugee or immigrant children preferred
