Job Description:


  • JOB SUMMARY
    • Reports to designated Manager, Population Health Care Coordination. The Community Resource Navigator (CRN) functions as a co-equal member of the care coordination team. CRN is responsible for identifying, high risk patients with socio-economic needs that impair their ability to comply with disease management instructions in order need to avoid readmissions and other adverse outcomes. The CRN will provide one-on-one guidance for patients to access health services, social services, benefits, or charitable resources in the community. As such, the CRN is expected to become a local expert on the array of community resources and services, acting as a representative for the Health System in community contexts, and maintaining an internal database of Community Based Organizations. The CRN will coach their patients on self-management strategies to maintain health and well-being independently.

  • JOB SPECIFICATIONS

    • Education and Experience
      • The knowledge, skills and abilities as indicated below are normally acquired through the successful completion of an Associate's Degree (Bachelor's Degree preferred). Some clinical training or college-level courses in a Human Services and or Social Work track are preferred. Certification as a Medical Assistant or other training is beneficial. A minimum of one year of experience in a healthcare or social services setting is required. Must have a valid State Driver's license.



    • Knowledge & Skills

      • Demonstrates understanding of community agencies, services and resources available to clients.


      • Requires basic knowledge of medical terminology and a familiarity with various insurance coverage options.


      • Requires self-motivation, good organization and time management skills.


      • Requires the ability to establish and maintain effective working relationships with clients/patients, families, staff, medical providers and the public. Must be able to establish a trusting rapport with patients.


      • Demonstrates the communication skills (both verbal and written) necessary to interact effectively with diverse populations; also requires the skills needed to communicate in a clear and effective manner.


      • Requires good listening and feedback skills, an ability to accurately assess difficult situations and respond accordingly, and a high level of cultural competency.


      • Requires the ability to use good judgment and maintain one's composure in any stressful situations.


      • Demonstrates proficiency in basic computer skills (i.e., data entry, word processing and spreadsheets).




    • Working Conditions

      • Works in an office and patient care environment.


      • Occasional possible exposure to communicable diseases and other conditions in a clinic or home setting.


      • Requires frequent travel to clients' homes, physician practices, community organizations, and other off-site locations.


      • Frequent face-to-face and telephonic communication is required.

      • Requires the physical ability and stamina to perform the essential functions of the position.
