This is a temporary work at home position and will eventually be working in the Pittsburgh office.
Hours: 10 a.m.-6:30 p.m. EST . Monday-Friday
Start Date: Must be able to start by August 2nd
Responsibilities
• Provide telephonic and/or digital customer nutrition-related support
• Empower customers to become an active participant in their own health outcomes
• Identify customer nutrition-related needs through targeted health assessments
• Provide customized diet based on diagnosis using the Nutrition Care Manual
• Utilize nutrition care indicators, biometrics, and motivational interviewing techniques to establish health improvement plans, set personalized evidence-based goals, and support customer in achieving improved clinical outcomes
• Be an active, motivated member on the interdisciplinary team; attend clinical rounds to discuss shared cases, establish next steps and accountable owners of action
• Collaborate with interdisciplinary partners through consults and referrals as appropriate
• Collaborate with internal and external matrix partners for care coordination
• Researches and refers customer to community resources (Diabetes classes, food assistance programs, food desert support etc.)
• Responsible for the adoption and demonstration of the Consumer Health Engagement cultural beliefs. Be a role model for the six cultural beliefs to drive personal accountability and organizational results.
Qualifications
• Registered Dietitian (RD)/ Registered Dietitian Nutritionist (RDN) with state licensure (as applicable)
• In good standing with the Commission on Dietetic Registration (CDR) required
• At least 3 or more years of direct nutritional care in a healthcare setting, providing health education and nutrition coaching
• Experience and/or familiarity with the Nutrition Care Process and Nutrition Care Manual
• Critical thinking skills to identify customer needs and recommend dietary care to meet their needs
• Ability to motivate customers towards health improvement
• Ability to work independently, be organized and focused, and effectively manage a daily customer caseload
• Excellent communication skills and time management
• Customer-centric focus
• Ability to proactively collaborate with clinical team, internal matrix partners and external client
• Understand and own a variety of clinical targets and outcome measurements
• Strong computer and software application skills
