Registered Nurse Care Navigator, GRACE Program, Full-Time, Days, Albuquerque, NM We are looking for a Registered Nurse Care Navigator to join our GRACE team. This full-time job is located with Lovelace Medical Group, a multispecialty provider group, which includes clinics, doctors and advanced practice providers in a broad range of specialties and primary care, and provides patients with exceptional health care through the support of Lovelace Health System hospitals and technology.This nurse role functions as part of the quality team and is actively engaged with the clinic health care team, partnering with the provider and staff to assure best practice transitions of care, proactive healthcare management and management of complex patients. Registered Nurse Care Navigator Job Responsibilities: Primary liaison for the continuity of care and social support systems for patients and their families throughout their health care experience. Develops education and training programs for patients, physicians and care staff on the appropriate clinical, community requirements and programs. Guides cancer patients through their course of clinical care working with a variety of medical practitioners, facilities, and community resources. Works with community advocates for the patient. Tracks and trends data for the program reporting requirements. Adheres to policies, procedures and regulations to ensure patient safety and compliance. Registered Nurse Care Navigator Education and Experience Jobs Requirements: Minimum: Bachelor in Nursing or equivalent required Master's in health related field preferred One to Two (1-2) years Navigator experience preferred Excellent communication skills Ability to communicate effectively Ability to work independently Basic computer skills Why Lovelace?
