The Case Manager is a member of the Outside The Lines (OTL) team. The program focuses on art and community activities that promote growth and independence. Case Managers work with the team to ensure individuals served by the program are supported in an environment that promotes independence and choice through the activities offered in the program.
Responsibilities/Duties
Oversees a case load of 6-8 individuals (may changed based on census)
Participates in meetings with individual/team to identify goals and strategies for reaching those goals as identified in the Individual Support Plan (ISP)
Prepares regular assessment and reports on goal progress
Works directly with individuals in the studio, one day a week works in office to prepare documentation
Develops, teaches and assists with classes and activities designed to address the needs and objectives of participants
Provides personal care to individuals in the studio as needed
Required Experience
A qualified candidtate will have some experience with the following:
Working with ID/DD/ASD populationFamiliarity with creative arts-based programming
Required Education
High School Dipoloma or equivalent - Associates degree or above preferred
Required Qualifications
Current CPR and First Aid Certification
MAP (Medication Admistration Program) certification preferred. Candidates without current certification will be provided with training but are expected to obtain certification within 3 months of hire
Valid Driver's License preferred, not required
