The Care Transition Nurse integrates clinical guidelines, protocols, and other metrics in the development of patient-centered transition plans, promoting quality and efficiency in the delivery of care. The Care Transition Nurse will represent the Agency in transitional care activities, strategic relationships with health systems, home health, hospitals, and other inpatient facilities, physicians, and physician groups. The Care Transition Nurse will serve as a public awareness representative for the Agency,

  • Performs a comprehensive nursing assessment and evaluation for the appropriateness of hospice admission.
  • Serves as an advocate for patients during the pre-admissions process and continuing until the patient is discharged from the medical facility.
  • Communicates to the patient, patient families, and/or RN to address options
  • Educates case managers on what makes cases appropriate for hospice services and the importance of transition at an appropriate time
  • Supports the Hospice service line in developing and maintaining sales and marketing plans, strategies, and public relations opportunities to promote and communicate the vision and mission of The Care Team.
  • Responsible for education relative to Hospice services provided by the Agency (Internally and Externally)
  • Monitor the execution of transitional care services through ongoing quality assurance visits with referral sources
  • Attendance at homecare Case Conferences
  • Partners with marketing staff at informational visits regarding hospice services
  • Assistance with hospice case managers when transitioning patients back to homecare when needed
  • Maintains patient confidentiality, including all HIPAA regulations.
  • Compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety, and emergency management.
  • Manages Territory with minimal supervision based on forecasted needs, priorities, and business strategy.

Professional Experience and Qualifications and Qualification


  • Must be a graduate of an approved school of nursing or social work and licensed in the state in which he/she currently practices.
  • Must demonstrate a strong understanding of customer & market dynamics & requirements, and most importantly, demonstrate a strong understanding of transitional care.
  • Must have at least one year of experience in the Hospice industry.
  • Knowledge of Home Health is preferred.
REQUIRED SKILLS/ABILITIES:

  • Utilize reports and trends to support efforts and to understand clinical status and progress as it pertains to patient care.
  • Data Analytics
  • Proficient with Microsoft Office Suite or related software.
  • Communicate effectively; orally and in writing.
  • Strong presentation skills
  • Interact well with a great diversity of individuals both outside and inside of the agency
  • Interact positively with all levels of personnel across all functions
  • Self-confidence and positive attitude toward self and others and maintain commitment and enthusiasm to goal achievement
  • Organize & execute programs
  • Prioritize multiple tasks & deadlines with minimal supervision.
  • Understand the Federal, State, and local laws/regulatory guidelines governing the operation of a Hospice and Home Health agency.
  • Driven by goals and growth
  • Self-starter who requires minimal supervision.
REGULATORY REQUIREMENTS

  • Must pass a criminal background check & MVR check.
  • Completed health statements acknowledging the ability to perform the duties of the position.
  • Valid state driver's license.
  • Proof of current automobile insurance.
  • Current CPR card; On-line CPR certification accepted; Hands-on CPR certification preferred.
  • TB testing per agency policy; (1 or 2 step TB skin test within 12 months of hire & annual TB symptom screening thereafter).
