The HARP Care Manager (Social Worker) is responsible for assessing the psychosocial and financial needs of CDPHP members and their families, arranging, coordinating, monitoring and evaluating ongoing member needs. The HARP Care Manager is also responsible for assisting HARP members identified at risk for recidivism, discontinuous care, or as members or priority or special needs populations who present with complex needs for coordination of their behavioral health service with other aspects of their care. The HARP Care Manager will assist these assigned members to care at all levels of the continuum, and provide any and all required field visits, discharge coordination, referrals to care plan of care development and care coordination as needed. A HARP Care Manager represents the organization to providers, member groups and families, and participates in the overall administration of clinical operations as warranted. This position will also provide assistance to others as called upon within the department.

The HARP Care Manager is expected to bring a level of clinical leadership to the care management department. In addition, a HARP Care Manager may serve as the care management lead for other members of his/her team. HARP Care Managers are responsible to meet with members in their homes, hospital and other outpatient settings.

QUALIFICATIONS:


  • Master's prepared mental health clinician, licensed in NY as LCSW, LMSW, LMHC or CASAC.

  • A minimum of three (3) years clinical experience is required.

  • Working knowledge of health, social service and funding resources, including community agencies and alternate care delivery systems is required.

  • Case management experience and CCM certification preferred.

  • Familiarity with community resources and social service resources to assist patients with social needs preferred.

  • Experience in home care, medical office setting or managed health plan preferred.

  • Certification in substance use disorders preferred.

  • Demonstrated ability to provide case management for complicated members with multiple comorbidities and chronic diseases.

  • Demonstrated ability to work with diverse population.

  • Demonstrates excellent clinical, written and oral communication skills, knowledge of clinical treatment, case management, and community resources.

  • Demonstrated ability to influence others while motivating them to change.

  • Ability to communicate effectively with members with varying levels of education, health literacy and understanding.

  • Demonstrated ability to interact with provider, members to positively impact the quality of patient care.

  • Possesses excellent clinical skills with sophisticated understanding of the overall needs of individual members assigned.

  • Proven customer service skills.

  • Demonstrated ability to work collaboratively in a team environment.

  • Demonstrated ability to review health care delivery against established criteria required.

  • Demonstrated ability to manage multiple projects in conjunction with patient caseload is required.

  • Demonstrated PC skills experience is required.

  • Ability to represent CDPHP in a professional and positive manner.

  • Flexibility to travel to a variety of sites across CDPHP service area required.

  • Access to reliable transportation to all CDPHP service areas to deliver service to members during regular business hours, evenings and possibly weekends is required.

  • Valid driver's license and driving record showing no restrictions that would impede ability to travel by automobile required.

  • Spanish speaking preferred.
