The Community Engagement Specialist provides services to Hope support recipients through their work in implementing a wide variety of recreational, artistic expression, and skill building activities and supports to maximize support recipients’ physical, social, and cognitive skills. The Community Engagement Specialist leads and supports children, teens, and adults who experience disabilities within recreational, artistic expression and skill building activities and teaches activities of daily living, as well as social and communication skills to foster social independence. This position implements goals and objectives as outlined in the Plans of Care and/or support plans designed to assist the support recipient in building skills useful to adapting to home, school and community environments. The Community Engagement Specialist also utilizes methods and techniques of teaching through which full participation in recreation, artistic expression, and skill building activities can occur. The Community Engagement Specialist maintains loyalty to the organization, its beliefs, values, mission, vision, and expectations and to the recipients of services.


 


Essential Job Duties


1. Provides services to individual or groups of support recipients simultaneously, while monitoring and teaching individual and group participation with an emphasis on skill acquisition associated with recreational, artistic expression, and skill building opportunities.


2. Implements, within the context of the recreational, artistic expression, and skill building activities, support services as outlined in the Plan of Care and/or support plan, including activities of daily living, respite, and day habilitation that develop and improve socialization, communication, education, social independence and community skills.


3. Ensures all services provided foster inclusive participation for support recipients in community and center-based activities.


4. Facilitates natural support connections within the community and assists support recipients in networking with community organizations, clubs, activities, and associations.


5. Collaborates and communicates with all members of the individual support team to assist with the coordination of services, development and implementation of support plans and goal directed activities, and the monitoring of their effectiveness.


6. Provides transportation for support recipients, including transporting through the use of assigned Agency vehicles and/or personal vehicles as assigned. Ensures proper usage and installation of car seats for safe transport when appropriate.


7. Assists support recipients with following medical orders and self-administration of medication; and provides medication administration and documentation, as specified by support recipient’s medical professional(s).


8. Completes accurate, timely, and thorough documentation, including but not limited to, service notes, goal tracking, incident reports, and timecards, to be in compliance with Agency standards and federal and state regulations.


9. Monitors all environments, agency vehicles, equipment, and events for safety, accessibility, and cleanliness.


10. Ensures proper use of all equipment. Ensures upkeep of supplies and equipment in all environments where recreational, artistic expression, and skill building activities occur.


11. Assists in implementation and instruction of recreational, artistic expression, and skill building activities. Coordinates activity locations as required.


12. Assists individuals with daily personal hygiene, dressing, toileting, and repositioning, feeding, as assigned or needed.


13. Utilizes and implements behavioral support plans effectively in group settings. Utilizes Agency on-call system in the event of an emerging crisis situation.


14. Works a variety of hours and locations and with a number of different support recipients in group settings.
