Job Description:
Thunder Care and Rehab is seeking an MDS Coordinator to join our outstanding team! This position requires an LPN or RN license. This position is responsible for resident assessments and care plans in accordance with state and federal guidelines. A professional and compassionate nurse in this role will make a positive impact in the lives of our residents.
- Must have an active State approved RN or LPN license
- Must meet all health requirements and pass background checks
- Experience in Skilled Nursing/Long Term Care setting required
- Experience with PDPM skilled assessments preferred
This is a full-time position.
