***This position will float to all Signature Health Pharmacy locations in Ashtabula, Cuyahoga and Lake Counties. Some evenings may be required.***


 


SCOPE OF ROLE


Reporting to the assigned site Manager, Pharmacy, the Pharmacist, Float is responsible for organizing pharmaceutical care for the purpose of achieving definite outcomes that improve a patient’s quality of life. You will effectively communicate with patients, prescribers, nurses and other clinical staff members when responding to medication inquiries. The Pharmacist will execute accuracy and attention to detail during the prescription filling process


 


HOW YOU’LL SUCCEED ____________________________________________________________________



  • Actively fulfill pharmacy needs by working at various Signature Health locations as scheduled by your Supervisor.

  • Interprets, clarifies, and manages the filling of physician orders using current pharmacy practice laws, acceptable pharmacy practices, available resources, and clinical judgment.

  • Performs prospective drug utilization evaluation with every order entry.

  • Maintains patient database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including over-the-counter and home remedies)

  • Monitors drug therapy and pertinent lab information/drug levels and provides appropriate interventions when required. 

  • Provide accurate and understandable pharmacy counseling and education to patients.

  • Maintain accurate and confidential records and documentation.

  • Response to medication inquiries in person or over the phone.

  • Provide excellent Medication Therapy Management and bubble packing of medications to patients when appropriate.

  • Assists in the coordination and implementation of quality assurance activities and assists with reporting and prevention of medication errors and adverse drug events.

  • Comply with all agency policies and procedures.

  • Maintain all aspects of HIPAA, promoting patient confidentiality and right to privacy.

  • May be required to perform telehealth services as determined by Signature Health.

  • Adherence and completion of compliance training provided by Signature Health.

  • Responsible for and completes all productivity/documentation requirements.

  • Participate in all assigned staff meetings and staff development programs.

  • Demonstrate appropriate attendance and punctuality in adherence with Signature Health policies.

  • Other duties as assigned.


 


KNOWLEDGE & EXPERIENCE ______________________________________________________________



  • Valid unencumbered Registered Pharmacist (RPh) License in the state of Ohio required.

  • 3-5+ years’ of pharmacy experience in clinical setting required.

  • American Heart Association (AHA) Basic Life Support (BLS) certification required.

WORKING CONDITIONS ___________________________________________________________________



  • Work is normally performed in a typical interior/office/clinical work environment.

  • While hours of operation are generally standard, flexibility to work evenings and extended hours may be required.

  • Requires periods of sitting, standing, telephone, and computer work.

  • Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology.

  • Speaking: adequate to speak to clients or patients in person, over the telephone or through telehealth technology.

  • Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.

  • Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted.

  • Possible exposure to blood borne pathogens while performing job duties.

  • Frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular workday.

  • Sufficient dexterity to operate a PC and other office equipment.
