Planned Parenthood of Orange and San Bernardino Counties has a full-time opportunity for a Integrated Care Coordinator in Anahiem, CA.
The primary role of the Online Behavioral Health Services (OBHS) Integrated Care Coordinator is to provide administrative and clinical support to the OBHS team by assisting clients in scheduling appointments, ensuring ongoing care through follow up calls, conducting brief phone screenings, identifying appropriate community resources, providing warm handoffs to other behavioral health programs and community resources, and monitoring program referrals and waitlist. The Integrated Care Coordinator may also provide administrative support to the OBHS team and perform other tasks as determined by the LCSW Manager. The Integrated Care Coordinator will work closely with a multidisciplinary team that includes primary care clinicians and physicians and behavioral health providers to ensure a seamless experience for clients and staff. This is a full-time 5-day/40 hours a week position that will be primarily remote, although there will be occasional in-person meetings.

Responsibilities
Serves as the first point of contact by phone for OBHS clients, and must be courteous, kind, and professional at all times.
Schedules active clients with assigned therapist and conducts follow-up calls for canceled or no-show appointments.
Conducts thorough phone assessments, including risk assessments, needs assessments, and program screening to determine appropriateness for services.
Returns patient calls in a timely and efficient manner if unable to answer immediately.
Provides appropriate referrals for clients needing additional services and maintains contact with clients until fully connected with a community resource
Researches, identifies, and builds relationships with other community resources by calling outside agencies, gathering relevant information for referral process, and maintaining list of active resources.
Manages waitlist and incoming referrals by identifying high-need clients, matching clients with available therapists, communicating with clients about the program, and updating referring physician/clinician on referral status.
Works collaboratively with OBHS therapists and Melody Healthy physicians and staff.
Provides administrative support to the OBHS team, including scheduling meetings, preparing agendas, and taking notes.
Qualifications
Minimum Education:
Associate’s degree required.
Bachelor’s degree in social work, psychology, or other related field strongly preferred.
Minimum Work Experience:
Minimum 1 year of experience in client or customer-service positions.
Behavioral Health Experience is strongly preferred.
Language Requirements/Preference:
Bilingual candidates are strongly preferred.
Other Requirements:
Strong written, verbal, and interpersonal skills.
Must be self-motivated and have the ability to work without direct supervision.
Demonstrated ability to multi-task and prioritize tasks with changing degrees of urgency.
