  • RN Continuing Care Coordinator-Sunnyview Acute Rehab Hospital -Per Diem

    What you will do:



    • Provide discharge planning and case management services to inpatients on assigned unit/program.




    Responsibilities:



    • Attend and participate in departmental meetings and in-services

    • Participate in any designated hospital committees as assigned by Department Manager.



    What you will need:



    • Associates in Nursing and NYS RN LicenseRequired

    • BS in Nursing or clinical practice preferred

    • Community Health experience and/or CRRN recommended

    • Strong organizational skills, verbal and written communication, assessment, and problem solving skills areessential.

    • Must have ability to interact positively with patients, families, community agencies, and providers
