Judge Healthcare is seeking a Case Management Coordinator for an opening with one of our top clients in Virginia!


Position Details:


Title: Case Management Coordinator


Type of Engagement: Contract


Rate: Competitive and Dependent on Experience


Location: Multiple locations around VA- up to 75% travel



Locations include: King George, Middlesex, Westmoreland, Lancaster, Northumberland, Essex, King and Queen, Richmond County and more


Duties:


1. Face to face visits in home


2. Responsible for assessing (in person), planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness.


3. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through integration.


4. Services strategies, policies and programs are comprised of network management and coverage policies.


Qualifications:


1. RN license


2. 1 years Case Management experience Virginia residency


3. Clean driving record


4. Valid Driver's License


5. COVID vaccination required


Preferred Qualifications:


1. Experience working within the health care Industry and with health care insurance


2. Experience working in a hospital, physician's office or medical clinic setting


3. A clerical or administrative support background


Preferred Qualifications


1. Managed Care Experience


2. Care Coordination/ Case Management in an integrated model


3. SNF/NF experience of greater than 1 year


4. Virginia resident


5. Master's Degree


Education Requirements:


Bachelor's degree in Social Work, Psychology ,Nursing or Health/Human services related field

Mon-Fri, standard business hours
